How to Enrol

Warnbro Primary School is a Department of Education designated Local Intake school, meaning the school provides places for all eligible students within our Intake area boundaries. Please refer to the Warnbro PS Local Intake Area Map.
Enrolment applications are carried out in the school office. Enrolments are not complete until the information below is submitted;
- a birth certificate
- ACIR (immunisations record, purple books are not acceptable)
- and proof of residence have been copied at the office.
Three pieces of proof of residency is required. These may include:
- A rates notice from the local council for the property
- A lease/rental agreement if the family is renting (with a minimum 3 months to cover commencement)
- Power – connection or latest account
- Gas – connection or latest account
- Contents Insurance
- Medicare statement
- Private Health insurance statement
If you are residing with family or friends on a temporary basis, further proof would be required. Please contact the school office.
Please Note: The Department of Education requires parents to advise the Principal in writing of any changes to their address and that enrolment may be reviewed and/or cancelled due to a change in residential circumstances. This is in accordance with section 16 of the Education Act.
Application and enrolments forms must be completed. Children must be enrolled under their legal surname i.e. the name on their birth certificate, or if changed, the deed poll must be provided.
Please be aware that the enrolment will take at least three days to process before the student can attend. On your child’s first day of school we ask that you come to the school office at 8.20am so you can be shown to the classroom to meet the teacher.
Your child’s immunisations are expected to be up to date on entry to school. These include measles, mumps, rubella, polio, whooping cough, diphtheria and tetanus. Immunisations can be obtained from your G.P. or contact Kwinana Community Health Centre on 9419 2266 for more information. We must be informed in writing if your child has not received their full immunisations, you will be informed of any outbreaks within the school and asked that your child not attend until it is deemed safe to do so.
If your child leaves the school, the school should be advised beforehand. Please ensure that your child takes all belongings from the classroom. Your child’s records will be transferred to their new school once proof of enrolment has been received.
Custody/Access Restrictions
Court Orders pertaining to parental access to children must be shown to the Administration/School office and a copy kept on file. This MUST be updated as required. If you have any concern in this matter please come to the school office.
Medical Action Plans
If your child has a medical condition or allergy/intolerance the school office MUST be informed. A medical action plan must be completed and kept updated as needed. Any medication will be kept in the office and is to be provided by the parent. If your child requires medication during school time please bring this to the office and complete an administration of medication form.
Updating Information and Emergency Contacts
It is extremely important to keep your personal information up to date with the school office. Your continued cooperation and consideration in this area will help to ensure that your child’s welfare will receive the highest priority, especially in times of emergency. All updates must be in writing and given to the school office to process.
PLEASE ENSURE THAT THE SCHOOL OFFICE IS KEPT INFORMED OF ANY CHANGE OF ADDRESS, MOBILE AND HOME NUMBER, FAMILY DOCTOR, EMERGENCY CONTACT, PLACE OF EMPLOYMENT, CHILDRENS HEALTH AND ALLERGIES, ETC.
